In-Home Nursing Care

At Inspire Hospice, we recognize that for many patients, receiving care from the place they call home preserves their independence and remain surrounded by their loved ones while ensuring they receive care according to their individual needs. That’s why our interdisciplinary team carries out in-home nursing care.

What Is Home Nursing?

In-home nursing care services are more than just companion care. They include much of the same medical and dietary care as a live-in hospice facility. But, instead of round-the-clock care, it is intermittent home visits by a nurse. This type of care is often provided for patients with limited mobility, inconsistent transportation access, or comfortable staying in their homes.

At Inspire Homage, our nurses collaborate with our care team of physicians, social workers, chaplains, and hospice aides to create a plan for in-home nursing care.

What Physical Services are Included with In-home Nursing Care

Our nurses will have a number of responsibilities that include coordinating patient care, communicating with the care team and the patient’s family, and the overall well-being of our patients.

In-home nurses can:

  • Schedule appointments
  • Make referrals to healthcare providers or specialists
  • Use medical equipment and supplies
  • Provide wound care
  • Manage prescriptions like picking up and reordering prescriptions
  • Monitor and prevent infections or bed sores
  • Make sure that the patient’s home is a comfortable and safe space
  • Provide rehabilitation therapy
  • Monitor changes in the patient’s behavior or condition
  • Monitor the patient’s vital signs and communicate these changes with the care team

Nurses also provide emotional support for our patients, whether it’s by advocating for the patient or just having an open ear.

Dietary Services with In-Home Nursing Care

In addition to physical care, our in-home nurses can help our patients improve their dietary habits by encouraging healthy eating. This is especially important for patients with diabetes or who are experiencing side effects from their treatment.

In-home nurses can also help by:

  • Educating patients about their diet
  • Help plan healthy eating menus
  • Assist with creating grocery lists

Who Pays For In-Home Nursing Care?

Generally, Medicare and Medicaid will cover the costs of in-home nursing care as long as patients meet the qualifying criteria. For example, Medicaid will have financial requirements, and only those who can prove they have a low income qualify for this coverage.

With Medicare, patients need to be:

  • Under a care plan that was created and reviewed by a doctor regularly
  • Cannot leave their house without considerable effort
